Online Form to Anonymously Report an Incident

This form allows you to anonymously report an incident with the option of making your identity known should you chose to formally report an incident. When you formally report an incident, you will be contacted by the Title IX Coordinator within 72 hours. Though you may disclose your identity for reporting purposes, that information will be kept private. If you decide to report to law enforcement or engage in the college internal investigation process, your identity will not be kept confidential.

At times, students may choose to seek the counsel of their advisor, instructor or department chair. Either of these college personnel may report the incident anonymously or confidentially to the Title IX Coordinator. The Title IX Coordinator will maintain the student’s privacy, unless the case will be adjudicated through the college’s internal investigation process or external law enforcement.

You are making an anonymous report, meaning we will not collect any identifying information from you. You will not be contacted and will remain anonymous.

This is not a system to use for emergencies. In case of an emergency please call 911.

  • Filing a police report means you will no longer remain anonymous and if you would like to do so, someone from the College can accompany you to the local police precinct to support you through the process.
  • Starting an internal investigation means you will no longer remain anonymous. The Title IX Coordinator will help the parties navigate the formal complaint process, access appropriate medical, emotional, or academic services, discuss possible interim arrangements during the investigation and resolution phases, and answer your questions along the way. The College will endeavor to promptly, thoroughly and impartially investigate, and resolve all complaints.

    Bank Street College has identified three providers to whom we refer students requiring immediate mental health support. Students should be in contact with their program director or department chair for a referral. We will pay for the first session with one of these referrals.

    SAFE & SANE Examinations—Preserving Evidence and Sexual Assault Exam:
    Survivors of sexual assault are strongly advised to have an exam completed by a SAFE (Sexual Assault Forensic Examiner) or sexual assault nurse examiner-adult/adolescent (SANE-A). Examinations of this sort may be difficult, so it is advised that, if possible, you have a friend or family member accompany you. In addition, the following are recommended to ensure the exam evidence is not contaminated.
